We provide comprehensive home assessments with a focus on safety and optimising independence. Changes to you everyday mobility or health  can affect many daily living activities eg. getting in and out of a chair or the shower or bed. Reduced mobility may also lead to difficulties accessing the community for appointments, shopping and social activities and can increase your risk of falling.

We can assess your performance of daily activities and make recommendations to make your life easier and safer. The recommendations are centred around you and your carer's goals as well as your individual circumstances. Professional OT reports can be provided to you or your case manager to support funding applications under the Home Care Package Guidelines.

Assistive Technology is a fancy word for equipment which is sometimes recommended to enhance your independence, increase your safety and make daily living activities easier. Assistive Technology can also reduce manual handling risks for family and paid carers. We can recommend a wide range of options depending on your individual needs and can organise trials and purchase if required. This can often save you the frustration and cost of purchasing unsuitable or unsafe equipment.

-Wheelchairs (manual, motorised, customised)

Making changes to your home can be a daunting process. Sometimes there comes a time when it can be beneficial to make changes to enable you to continue living independently. Home modifications may also reduce your risk of moving into residential care. Our involvement can include concept drawings, advice on disability building standards based on AS 1428.1-2009, organising contractors, quotes and small scale project management.

Service Criteria

Client is over 60 years of age.

Client is agreeable to referral.

Payment via Aged Care Package funding only.

Medicare, DVA and NDIS funded clients are not currently eligible.

​

We work with clients that have wide ranging health conditions eg. Orthopaedic, Neurological, Cardiac, Respiratory, Post Surgical, Dementia, Chronic Pain, Disability and Palliative Care.
